It sets out to explain your individual rights, such as your right to know what data is held about you, what we do with your information and the choices you have over your personal information. This includes data collected when accessing this website, contact via email or telephone and during therapeutic appointments.

WHAT IS PERSONAL DATA?

Personal data is personal information relating to an identified or identifiable individual. Personal data includes an individual’s name, age/date of birth, gender, contact details e.g. home address/telephone number/email address, GP details and emergency contact/next of kin information. Personal data may also contain information known as special categories of personal data. This may include sensitive information relating to employment, sexual orientation, religion, family background, social circumstances, medical and mental health information.

In accordance with General Data Protection Regulation (GDPR) guidelines, we only collect information that we genuinely need and use.

HOW WE USE YOUR INFORMATION

We use the information you provide us in the following ways:

  • To contact you if you have made an enquiry or requested contact as a potential client

  • To provide you with relevant information that you have requested from us

  • To provide psychological assessment and treatment

  • To communicate with you prior to or following appointments, in order to schedule appointments or notify you of any changes

  • To notify you where appropriate and to complete any administrative, legal or ethical obligations

  • To provide you with information of relevant services that we think may be of interest or useful to you as part of or as an alternative treatment option

  • To request feedback following assessment and/or treatment as part of service development

  • When you visit our website

HOW YOUR INFORMATION WILL BE SHARED

We may share your personal data with others in the following circumstances:

​

  • If the law or public authority requests us to share personal data

  • If you have provided us with consent to share relevant data with medical professionals such as your GP/Medical Practitioner, other professionals such as social care, solicitors, medical insurance companies etc. or others you have agreed or requested information to be shared with e.g. family members

  • Where we are concerned of serious harm to yourself or others (this may be without your consent)

  • During supervision we may share details of a clinical case for case discussion for professional development. Supervisors with be accredited by their professional body and work within their own ethical framework

HOW YOUR INFORMATION WILL BE KEPT SAFE

Data security is of great importance within Therapy Solutions in Mind.

​

  • All information provided will be stored securely under GDPR Guidelines

  • Brief therapy notes may be made prior to therapeutic contact, which will be kept separate from any identifiable personal information such as name and address

  • Any written notes or manual data will not be left unattended and always stored in a locked secured location

  • Personal data that is sent via email, including letters and reports, will be sent via an encrypted email provider

  • Any known breaches will be reported to the Information Commissioner’s Office (ICO) within 72 hours and you will also be informed

  • Mobile phones that hold your telephone number will be stored anonymised (initials only) and a secure pin protects the phone and information

  • We retain a record of your personal information in order to provide you with a high quality and consistent service. We will always retain your personal information in accordance with the General Data Protection Regulation (GDPR) and never retain your information for longer than is necessary

  • If you are a client, then we are required by law to keep your clinical records for a period of 7 years after our last contact with you at which point they will be deleted/ shredded. For children this will be until the age of 25. In some circumstances we may anonymise your personal information so that it is no longer associated with you i.e. clinical outcome data, and retain this indefinitely in order to monitor and evaluate our service. If you are not happy for us to do this then please let us know

 

YOUR RIGHTS

In accordance with the GDPR and the Data Protection Act (2018), you have the right to:

  • Be informed about the collection and use of your personal information

  • Access your personal information

  • Rectify, restrict, erase or object processing of personal information

  • Data portability (to receive an electronic copy of your personal data for personal use across difference services)
     

Individuals can exercise their individual rights at any time. However, if you object to the collection, storing, use and sharing of personal information, we may not be able to provide you with a service.
